# Insurance Cards

**You can either:**

**1. Take photos now** - Tap **Take Photo** to open your camera and snap the fronts and backs of your cards.

**2. Upload existing photos** - If the pictures are already saved on your device, use **Browse Files** to upload them.

- Take a photo of the front of your primary insurance card.

- Take a photo of the back of your primary insurance card.

- Take a photo of the front of your secondary insurance card (if applicable).

- Take a photo of the back of your secondary insurance card (if applicable).

- If you already have photos on your device, please upload pictures of the front and back of both your primary and secondary insurance cards.
