Insurance Cards

You can either:

1. Take photos now - Tap Take Photo to open your camera and snap the fronts and backs of your cards.

2. Upload existing photos - If the pictures are already saved on your device, use Browse Files to upload them.

• Take a photo of the front of your primary insurance card.

• Take a photo of the back of your primary insurance card.

• Take a photo of the front of your secondary insurance card (if applicable)

• Take a photo of the back of your secondary insurance card (if applicable)

• If you already have photos on your device, please upload pictures of the front and back of both your primary and secondary insurance cards
