Humana has teamed up with Carda Health to provide an online cardiac rehab program that can speed up your recovery from home. This program is available at **no additional cost through your Humana plan**.

## What is Pulmonary Rehab?

If you've recently been hospitalized with COPD or a breathing issue, Carda may be able to help you recover and improve your health so you can get back to doing the things you love again.

Carda Health’s virtual pulmonary rehab program is provided to qualifying Humana members at no additional cost.

### Pulmonary Rehab

Pulmonary rehabilitation (PR) is **a medically supervised program that helps people with chronic lung diseases live and breathe better**. This is often done through breathing exercises, physical activity, and vitals monitoring of blood oxygen levels.

### Virtual Pulmonary Rehab

Virtual pulmonary rehab is similar to in-person pulmonary rehab, but conducted online _at home instead._ You can do all the same exercise programming from the comfort of your home. The program is intended to help patients with **COPD, Asthma, or Pulmonary Hypertension.**

Carda Health’s virtual pulmonary rehab program includes guided breathing exercises, education, and counseling that may help improve lung health. All equipment is provided and sent right to your door.

## How it Works

1. **Answer the call from Carda**  
   If you’re an eligible Humana member, you’ll receive a call from Carda Health to introduce you to the program. Or, call Carda to learn more.
   
2. **Get Your Carda Care Package**  
   No need to worry about devices, we’ll ship you everything you need to participate in the program, and walk you through the set up process.
   
3. **Meet Your Coach**  
   Get matched with a clinical exercise physiologist who will design your program to help you meet your personal goals.
   
4. **Get Back to a Healthier You**  
   Meet your coach by video three times a week for 12 weeks. Get support to achieve your goals like feeling stronger, breathing easier, or doing the things you love again.

## Frequently Asked Questions

### What are the sessions like?

When it’s time for a session, you simply tap a button on your iPad. You’ll take your blood pressure and heart rate, then meet with your coach to talk about your progress and share any questions you have. Your coach will start an exercise video that’s been selected to give you just enough of a challenge, without pushing you too hard. You’ll exercise while your coach monitors you by video and tracks your breathing. After the session, you’ll discuss any other needs you have, including your diet, medication questions or health education. A session can take anywhere from 30-45 minutes.

### How long is the program?

The program is 36 supervised sessions, with 45 minutes for each. Typically, members participate in sessions 3 times per week, but Carda Health works with you to create a program that works well with your schedule.

### How does pulmonary rehab help? Why is it recommended?

Pulmonary rehab can have many health benefits in the short and long term, such as:

### Am I healthy or strong enough to do pulmonary rehab?

Pulmonary rehab will meet you where you are. You may start with seated exercises, while a live coach is always there to monitor you and suggest modifications, if needed. Carda Health coaches regularly check your vitals to make sure you’re exercising safely, and only doing exercises that you feel comfortable doing.

### How do you monitor my progress? What do you monitor?

Carda Health reviews your progress via the Carda Health app. Your dedicated personal coach, who is a trained clinician, will monitor your heart rate, blood pressure, and blood oxygen during every session and evolve your program as you make progress.

### How do I enroll?
